Physical Key Requests

Use this service to request a physical key for campus spaces, arrange a key return, or report a lost or stolen key. Requests are reviewed by Facilities and may require approval before a key is issued.

Please note: this form is for physical keys only. It is not for access card or alarm system requests.

After you submit, Facilities reviews your request and confirms the appropriate key or solution. If approval is required, your request is routed for sign-off before a key is issued. You will receive email updates at each stage — including when your key is ready for pickup. Along with the pickup notification, you will receive a Key Terms email — you must accept and agree to the key terms before the key can be issued to you.

Before you accept your key: review the Key Terms email and reply to accept and agree to the terms. The key cannot be released until the terms are accepted — they cover the responsibilities listed at the bottom of this page.

Requesting a Key

Include in your request:

  • Building and room/door the key is for
  • Reason for access — your role and why you need the space
  • Any other details that help Facilities confirm the appropriate key or solution

Returning a Key

Keys must be returned to Facilities when they are no longer needed or when you leave your role. Bring the physical key with you so it can be checked in against your record — you will receive a confirmation email once the key is checked in, and your access associated with that key ends.

Key Holder Responsibilities

Once a key is issued to you, you are responsible for it:

  • Accept and agree to the key terms before accepting your key.
  • Keep it secure at all times.
  • Do not lend, copy, or transfer the key to anyone else.
  • Return the key to Facilities when it is no longer needed or when you leave your role.
Lost or stolen key? Report it to Facilities and/or Security immediately.
