Continuing Studies Certificate Reprint Request

Requesting a Certificate Reprint

Use this form to request a reprint of a Continuing Studies certificate that has already been issued. Reprints may be requested if a certificate has been lost, damaged, or requires a name update. A replacement fee of $50 applies.

Certificates are processed and printed twice per year. Once printed, certificates may be mailed to the address you provide or made available for pickup on campus.


Submission Deadlines

Deadline Processing Timeline
Third Thursday in May Ready mid-June
Third Thursday in November Ready mid-December

Late submissions will be processed during the next production cycle.


Processing Time

Please allow up to two weeks for mailing once certificates have been sent.
Certificates selected for campus pickup will be available within the same timeframe.


Who can use this service

This service is for students and alumni requesting a reprint of a certificate that has already been issued.

If you have not previously received a printed certificate for the program, please use the Continuing Studies Certificate Request form instead


When to use this form

  • If your certificate has been lost or damaged
  • If you require a reprint due to a legal name change
  • If you have already received a certificate for the program

Note: If you are requesting a reprint due to damage or a name change, the original certificate must be returned to Continuing Studies before a replacement can be issued.
